Faculty and Staff can use the Equipment Activity Form to report changes in the status and location of equipment, while certifying compliance of digital media policy 5-06. The equipment cannot be disposed or relocated until the originator has received an approved copy of this form. Equipment actions include: disposal, location change, fabricated equipment, found equipment, equipment loans, and digital media destruction certification.
